COVID-19 Test Kit Claim Policy

The Northwest Laborers-Employers Health and Security Turst adopted a temporary plan change beginning Jan. 15, 2022.


This was to comply with a new Federal mandate and will last until the end of the National emergency for COVID-19.


The cost of at-home COVID-19 test kits that are purchased for individual diagnosis of COVID-19 will be covered by your plant 100 percent, upt to a maximum of 8 tests per covered person, per month.



  • Claims for reimbursement must be made by completing a COVID-19 found here: Over-the-Counter Test Kit Claim Form.


    • Attach a receipt that shows the UPSC code and date of purchase

    • Mail to:

    • You can also faxt it to (206)352-6975 (put "Attention: Northwest Laborers" on your fax)




EXCLUSIONS: At-home COVID test kits used for employment related testing ARE NOT a covered expense.
COVID-19 test kits purchased on third-party reseller on-line plaforms (such as Craigslist, Offer-Up, Facebook Marketplace, eBay, or
Amazon third-party resellers are not eligible for reimbursement.
